Anal Fissure - Treatment

What is Anal Fissure?


Anal fissures are quite a common condition which is described as a tear in the inner lining of the anal canal or anus. This is equally common in both men and women. Anal fissures are often misdiagnosed as hemorrhoids which is different, wherein the veins in the anal canal are swollen or becomes tortuous.


Causes of Anal Fissure?The most common cause of anal fissures is constipation. Patients suffering with constipation for a long time have more chances of developing anal fissures. An anal fissure develops when the resting pressure of the internal anal sphincter becomes too high resulting in reduced blood flow which in turn results in a fissure.

Other causes that may lead to above mentioned pathology are as follows:

  • Repeated Diarrhea

  • Childbirth can cause injury to the lining of the anal canal.

  • Irritable bowel syndrome.

  • Passage of hard stools can cause cracks or fissures.

  • Anal intercourse, Foreign body insertions.

  • Digital examination of the anal canal can cause fissures if not performed correctly.


Symptoms:Pain is the hallmark symptom of a patient suffering with anal fissures. The pain is usually described as a stitching/stinging/burning pain and this may last from few minutes to many hours.

Itching is the second most common symptom and this itching sensation may occur either before or after stool.

Bleeding per rectum occurs often when hard stools are passed and it can be seen as streaks of blood on the stools passed. The patient may have severe bleeding per rectum, if the fissure breaks open even more than before while straining at stool or exercising or during a digital examination. The bleeding is usually bright red and sometimes it may be dark red which indicates that there may be some other associated conditions like irritable bowel syndrome or colonic cancer.

In some patients it can be noted that they have a painless anal fissure with severe bleeding. The pain may be absent due to prolonged use of local analgesics or painkillers.


Diagnosis:The diagnosis is usually made from the symptoms and signs. On inspecting the anal orifice we can find a skin tag (sentinel tag) which a diagnostic sign of anal fissure which may look like a hemorrhoid (a haemorrhoidal mass will have a mucosal lining and will not be covered by skin). A digital examination is rarely performed in a patient with fissure as it may be very painful for the patient. In such cases an anoscope or a proctoscope may be used to confirm the diagnosis and local anesthetic will be used.
