What Causes Eczema?
The exact cause of Eczema is still unknown but there is increased incidence in patients with a family history of asthma and other skin disease. Epidermal barrier impairment is another reason why it run in some families genetically. Epidermal barrier impairment causes decreased moisture content thereby allowing the germs in.
Environmental factors like exposure to allergens in utero or during childhood may also be the cause. 60-80 % of the individuals are genetically susceptible to the induction of IgE Medicated sensitization to environmental allergens such as food and animal hair.
Decrease in skin barrier- allows greater penetration of allergens through epidermis – Causes immune stimulation -> resulting in inflammation Psychological factors like stress and depression can be contributing factors to trigger eczema.
Types of Eczema:
Atopic Eczema: This is caused due to hypersensitivity to a common environmental antigen such as pollen and house dust mite etc in which there is a genetic disposition to produce excess IgE. Its prevalence has increased 2-5 fold since early 1980s and the disease now affects 1 in 10 school children.
Seborrhoeic Eczema: This almost resembles psoriasis which presents with erythematous scally rash of the scalp, eyebrows, nasolabial folds chest and upper back. Patients usually think that they are suffering with simple dandruff but as the scaling becomes excessive they get worried and consult a healthcare practitioner.
Discoid Eczema: This type of eczema is predominantly found in adults which presents with coin shaped lesions which is very itchy with scaling.
Irritant Eczema: This type of eczema is caused due to prolonged exposure to chemical agents , strong acids, alkalis etc.
Allergic Contact Eczema / Contact Dermatitis: This type of eczema usually occurs at the site where the allergen is in contact with the body like eczema of earlobes dues to earrings ( nickel ear rings), wrists due to watches or bracelets, neck due to chains or hands due to use of rubber gloves etc.
Asteatotic Eczema : This a commonly found in elderly. Low humidity caused by central heating, over-washing, diuretics and cholesterol lowering drugs predispose.
Gravitational (Stasis) Eczema: Usually occurs in lower limbs and is often associated with signs of venous insufficiency.
Lichen Simplex: This usually occurs due to constant rubbing, scratching which leads to lichenification of the areas.Pompholyx: These present with intense itchy vesicles and bullae on the palms and soles.
